Irene Brady, the publisher of Illustrating Nature, Redrock Canyon Explorer, and five other titles put out by Nature Works, has kindly agreed to share a copy of the CIP block I did for her to serve as an example for people who don't understand what a CIP block is. You can also see many other examples if you look on the copyright page of many books.
I send CIP blocks electronically. You may change the formatting to make it centered, change font, change size of print, etc., but you MAY NOT change the content or change the grammar of the block.

(1) Publisher's Cataloging-in-Publication Data. This tells the librarian that it is a CIP block supplied by the publisher.
(2) This is called the Main Entry. Typically it is the author's name and it is one way in which the book is indexed in the Library of Congress's catalog.
(3) Title statement. Notice that only the first word is capitalized. This is an idiosyncrasy of the way CIP blocks are written. The only words in the title statement that are capitalized are the first word and any proper names in the statement. Don't tell me I made a typo - I did this on purpose!
(4) Statement of responsibility. This is written exactly as it appears on your title page.
(5) This is the physical description of the book, but it's typically left blank. I don't know why! Once librarians have the book in hand they may fill in these details themselves.
(6) Statement of index and/or bibliographical references.
(7) International Standard Book Number. If the book has an ISBN 13 or two ISBNs (one for hardcover and one for paperback, for example) they will both go here. You need an ISBN if you want to sell your book through bookstores or distribute to libraries. You can get a block of them through Bowker.
(8) Subject headings. These are chosen based on authority records already in the Library of Congress database. They cannot be made up and I cannot include records that don't already exist.
(9) Additional entries. This means other ways your title will be inputted into the library's databases. In addition to author, it will be inputted by title. It could also be inputted through a second author's name, if that applies.
(10) Library of Congress classification number. In this case it puts the book next to other books on natural history illustration. LC classification is used by university libraries and other large library systems.
(11) Dewey Decimal classification. This also puts this book next to other books about natural history illustration. The Dewey system is used by public and school libraries and some small college libraries.
(12) This is the PCN (Preassigned Card Number) or LCCN (Library of Congress Control Number). Because you are seeking my services, it's most probably a PCN. You can apply for a PCN through the Library of Congress's PCN program. It's free and it's an important part of the CIP block, so I recommend that all my clients get one for their book. It provides another reference point for librarians to use when they want to find your book.
